1. Call to make an appointment (415) 523-4939 or email us at hello@missionbaydental.com. If you plan to use your dental insurance benefits, be sure to have your insurance information available, including insurance company, group number and member ID.

2. We will send you a HIPAA compliant link to complete your new patient intake forms to complete prior to your appointment.
